Milan match shirt issued/worn by Andriy Shevchenko for one of two UEFA Champions League matches during the 2003–04 season: Club Brugge vs Milan on 4 November 2003 or AC Milan vs Sparta Prague on 10 March 2004.

The shirt can only be attributed to these two fixtures, as they were the only matches in which Shevchenko used the long-sleeved home version of the shirt.

The UEFA Champions League group stage match between Club Brugge and AC Milan ended in a 1–0 victory for the Rossoneri. The second match, the UEFA Champions League Round of 16 second leg against Sparta Prague at San Siro, finished 4–1 in Milan's favour, with Shevchenko scoring twice.

The shirt features an inner mesh lining.
